Check if the GitHub 5xx circuit breaker is currently open.

When open, non-critical background work (cleanup, polling, mention scans) should be skipped. Critical operations (PR creation) should still attempt with their existing exponential backoff.

Automatically recovers (logs a recovery message) when the cooldown expires.
