Create a global token resolver from configuration.

Resolution order:

  1. GH_TOKEN / GITHUB_TOKEN env vars
  2. gh.auth.token config value
  3. gh auth token CLI (unless gh.allow_gh_fallback = false)

This is typically called once at application startup.
