Render a ContextReport as the digest an assistant reads: at most MAX_CONTEXT_LINES lines, byte-identical for the same report.

A report carrying no source — what context_with answers by default — is rendered as a pointer instead: at path:start-end, the signature, and the graph’s facts. Nothing stands in for the missing body, because a pointer is not a truncated body; it is the whole answer to where the body is.
