Parse a helpcode table (helpcode_utils.cpp:54-88): BOM and \r stripped, # lines skipped, left=right with the first up-to-two a-z letters of right, later lines overwrite. A missing custom file fails with UNKNOWN_HELPCODE_SCHEMA.
