Schema bootstrap and versioned migrations.
The if version < N blocks in apply_schema_migrations are FROZEN TEXT:
never reformat, reorder, or “clean up” historical migrations. The upgrade
path from every deployed version must replay byte-identically. New schema
changes append a new version block and bump SCHEMA_VERSION_CURRENT.
