Typed questions and their rendering into an encoder sequence.
Layout, matching build_sequence in rl_common.py:
[CLS] <type> question: <instructions> [SEP] [MASK] opt0 [MASK] opt1 ... [SEP] <state> [SEP]
Each option is preceded by a [MASK] marker; the head scores those marker positions.
