Pair only markers with the same prefix, stream, suite and span label.

This prevents a start from one prefix/stream consuming another’s end. Order within each origin remains caller-supplied and nesting remains last-start-first. Prefix/stream are grouping facts, not authenticated producer identities; callers must keep independent producers with otherwise equal fields separate. Returned spans and subsequent aggregation retain their existing suite/label shape.
