Fully inline already-prepared refs without allowing file/URL retrieval.

This is the final-output counterpart to flatten_refs. Input assembly is responsible for fetching and preparing external refs. If an external ref reaches this pass, the schema is not self-contained and the run fails.

Returns CliError::Referencing when a reference is malformed, unresolved, or still points to an external document, or an error when base_dir cannot be represented as an absolute file URI.
