Decode a BinHex 4.0 (.hqx) container into both forks plus Finder info.
Unlike the sidecar formats this is a whole-file wrapper, so data_fork and
name are both carried and the caller writes those instead of the .hqx
text. Returns None when the document does not decode or carries neither a
resource fork nor Finder info, so a plain BinHexed text file still imports
as its own contents rather than as an empty-forked stub.
