Render page index into a width × height bitmap the way pdfium’s FPDF_RenderPageBitmap (white-cleared, FPDF_ANNOT) would, if the page is image-only in the sense above. None — with the reason under DOCLING_RS_DEBUG — when it is not, or something in it is unsupported.
