Parses CSV records from any BufRead with explicit options.
The delimiter and quote bytes must be ASCII and differ. A trailing newline
is optional; fully blank records are dropped, and a header-only file is an
EmptyInput error. Each parsed record also carries the 1-based physical
file line where it started (blank lines and quoted newlines are counted).
