Outcome for an action that the activate tier performed.

outcome reports focus_restored: Some(true) for every activate via, which is right on macOS (that tier puts the previous app back). Windows activates an element pattern as a retry, and passes what its own SetForegroundWindow restore actually achieved, so this constructor takes the flag instead of assuming it.
