Pure inner comparison for the max_findings_in_hot_files gate.

Counts "act-now" rows in rows; returns a single repo-wide violation when that count exceeds threshold. actual is the act-now count; path is "(repo-wide)".

Called from the CLI layer after opening the external store and running run_finding_hotspot_overlap. The gate is skipped (not evaluated at all) when the store is absent or empty — that skip path lives in the CLI layer and does not go through this function.
