PostgreSQL-backed Journal for Cairn.
Appends are atomic compare-and-set on the run’s next sequence number, so
any number of processes and machines can drive, resume and approve the
same runs: exactly one writer wins each position, the rest get
JournalError::Conflict and can reload and retry.
