Extract the names of failing tests from a runner’s output.
Recognises the shapes cargo test, pytest, go test, and the jest
family emit. Deterministic, in the order encountered, without duplicates.
Best-effort by design: an unrecognised runner yields no names and the failure reason falls back to the tail of the output, which is still useful. Guessing wrongly would be worse — a repair loop sent after a test that did not fail wastes an attempt and an inference call.
