Decide whether to ask the queen and, if so, queue the questions. Returns the asked entries
(empty: nothing asked). Errors only on I/O; the caller fails open. Cheap checks first: the
lessons file is read only when a candidate decision survives them and the budget. A question that
near-duplicates one already queued for its repo (duplicate) is not queued again. Newly queued
questions get one desktop notification (notify.rs).
