Module guard 

Loop guard — prevents runaway agent loops (Jidoka pattern).

Tracks iteration count, tool call hashes (FxHash ping-pong detection), cost budget, and consecutive MaxTokens responses. Verdicts: Allow, Warn, Block, or CircuitBreak. See arXiv:2512.10350 (Tacheny).
