Per-row file score tuple shape used by the prepare-change pipeline. (score, file, layer, last_touched_days, hot, top_symbol, why)
Kept as a tuple (not a struct) because the t-009/t-003 sites already
destructure it positionally in many places. The longer-term refactor
can promote this to FileScoreEntry (which exists above) once all
call sites use field access.
