Evaluates a call against every applicable guardrail.

Guardrails run in order and revisions compose: a later guardrail sees the arguments an earlier one produced. Evaluation is sequential rather than parallel because a revision has to be visible to whatever runs next — parallel evaluation would make the outcome depend on completion order. The first denial stops evaluation, so a denied call is never revised and never reaches a later guardrail.
