Unified Safety Gateway

Consolidates all safety checking mechanisms into a single gateway:

• Rate limiting (from runloop’s tool_call_safety)
• Destructive tool detection
• Command policy enforcement
• Plan mode restrictions

This provides consistent safety decisions across all tool execution paths.
